Safety Observation

Report Un-Safe Act / Condition

Objective : It allows user to EHS Observations of unsafe act / unsafe condition in any GA in the system.

Below steps to be followed to complete a EHS Observation report :

  1. Report Unsafe act / condition Observation : To create new EHS Observation. Observation shall be done and reported by any system users.
  2. Review Unsafe act / condition EHS Observation :
    • “GA Head”/”HSSE manager” will review observations once it’s reported.
    • If observation not valid (i.e. duplicate observation reported), then “GA Head”/”HSSE manager” can discard those observations.

Note :

  • Any of the power can report can do EHS Observation

  1. Navigate to EHS portal | Unsafe act / condition | Report unsafe act / condition screen. (Image 1)
  2. Select GA, Asset type, location and enter the title, Exact Location.
  3. Select Observation category and provide observation detail.
    • Unsafe Act
    • Unsafe Condition
  4. Click “Submit” button to save the data and send the observation report for review to GA Head / HSSE Manager.

Note :

  • The fields marked with * are mandatory to be filled.
  • Observation shall be reported by any system users.
